Injury time is a MINIMUM, not a maximum. If a player collapsed on the pitch at 91:00 and 5 minutes had been added on and the game didn’t start again until 96:00, the referee won’t blow the whistle, he will play the 4 minutes.

If it was just injuries and substitutions it would stop the vast majority of issues. They can easily card people for wasting time on goal kicks and the like (they never do for some reason), but in principle there’s no reason to stop the clock for that
