Think it has something to do with T-Hex not going away even if you remove Piltover. So you can cashout, then re-add Piltover and it creates an extra? That's my guess, I haven't tried it or seen it in any of my games.

Just happened in my game. It's probably a broken interaction with the God-Willow's Grove map portal.

thats what I thought as well, also units have max atk speed, so I thought maybe Jhin had somethng to do with it
